Corrections

Material changes to clinic records receive a dated note.

  1. Identify the field

    A report should name the clinic record, exact field, current text, and a public clinic-owned or official source supporting the requested change.

  2. Recheck the source

    The source is checked against the published field and its date. Conflicts stay unresolved until an acceptable source establishes the replacement.

  3. Update the record

    A material correction states the changed field and date. Routine rechecks that do not alter meaning update the field date without a correction note.
